Basic demographic information for you and your child. This includes full names, child's date of birth and grade level, phone number, and email address.

Desired start date. All start dates are on Mondays, and your program will span 24 weeks from your chosen start date. You will receive an email after registration with a link to schedule your intake appointment during your first week, as well as instructions for scheduling your one-on-one tutoring.

Payment method. You will be required to select your payment method: SUFS direct pay, reimbursement by another scholarship, or private pay. If you select reimbursement or private pay, you will be required to submit payment in full at the time of registration and you will be forwarded to PayPal upon submitting the registration form. If you select SUFS direct pay, you will not be forwarded to PayPal; you will receive an email from us once we have verified your order through EMA.
